Kindly attach a clear copy of the full page of the bank certificate. Must be issued within 3 months valid from the date of issuance. You must indicate the ADB in the bank certificate.
* 3 months bank statement is required if applicant has No ITR and Bank Certificate has No ADB 

Kindly attach a clear copy of full page of Income Tax Return Certificate (BIR Form) - It must have amount of gross income and a stamp of recipient,
          (1) For person subject to income tax withholding, the Certificate of Compensation Payment/Tax Withheld (BIR Form No. 2316) must be signed by the employer and employee. 
            (2) For taxpayers other than above, the Annual Income Tax Return (BIR Form No. 1701, etc.) must have a receipt stamp of the tax collector.
            (3) If tax is not withheld (e.g., self-employed, housewife, retired) and if the Average Daily Balance (ADB) is not stated in the Bank Certificate, please submit Bank Statement to prove transaction for the last 3 months

Kindly download the Guarantee letter form, print it on A4 size paper, and fill out the needed details. Then upload it to the visacenter.ph app. A signature is not required in the Letter of Guarantee form. Download here

* Parents must sign on behalf of applicants who are 12 y/o and below. Please specify the relationship beside the signature.
* 13 and above, applicant must sign all forms
